If your eyes look more tired than they feel, you may be wondering what type of treatment could make the biggest difference. Many of our patients here at Columbia Aesthetic Plastic Surgery ask whether BOTOX alone is enough, or if eyelid surgery is the better option.
The answer depends on what’s causing the change you’re seeing and which areas bother you most. Surgical treatments like eyelid surgery and nonsurgical options like BOTOX play different roles, and sometimes the best result comes from combining them. Here’s how to decide what might be best for you.
What BOTOX Can Improve Around the Eyes
BOTOX works by relaxing the muscles that create expression lines. Around the eyes, it’s commonly used to soften:
- Crow’s feet
- Frown lines between the brows
- Horizontal forehead lines
It can also create a subtle lifting effect along the outer brow. For some people, this small change opens the eye area enough to create a more refreshed appearance without surgery. If the concern is mostly movement-related wrinkles or early brow descent, BOTOX alone may be an excellent option.
What Eyelid Surgery Can Address
Eyelid surgery treats structural changes that BOTOX cannot correct. Upper eyelid surgery removes excess skin that can create hooding or heaviness. In some cases, hooded upper eyelids can be so severe that they impair vision. In this case, eyelid surgery would be the better solution compared to BOTOX. Lower eyelid surgery addresses puffiness from fat pads or loose skin beneath the eyes.
These changes are common over time and often contribute to the “tired” look that our patients notice, even when they’re well rested.
When extra skin is the primary concern, relaxing nearby muscles won’t fully resolve the issue. In these situations, eyelid surgery typically provides a more complete improvement.
Why Injectables Alone Aren’t Always Enough
One of the most common misconceptions is that injectables can replace surgery around the eyes. While BOTOX improves wrinkles caused by muscle movement, it does not remove excess skin or reposition fat beneath the eyes. If hooding or under-eye bags are present, injectables alone usually won’t create the change patients are hoping for.
This is often the point where our patients begin to consider eyelid surgery as the next step.
When Combining Treatments Makes Sense
In many cases, the most natural-looking result comes from using both approaches together.
For example, upper eyelid surgery can remove excess skin, while BOTOX softens the surrounding expression lines that contribute to a tired appearance. Treating both structural changes and muscle activity creates a more balanced improvement across the entire eye area. This combination often helps the eyes look brighter without looking different.
How to Decide What’s Right for You
The decision between BOTOX, eyelid surgery or a combination of both depends on your specific goals and facial anatomy. If the concern is mostly fine lines or early brow drooping, BOTOX may be enough. If excess skin or under-eye fullness is present, eyelid surgery may be the more effective solution.
A consultation with Dr. Chang or Dr. Wang will help to determine which approach best fits your anatomy and your goals. To get started, please call our Columbia, MD office at 410-740-9330 or contact us online.
