What Is Eyelid Surgery?
Eyelid surgery, medically referred to as blepharoplasty, is a surgical procedure that removes excess fat, skin and muscle from the upper or lower eyelids, or both. While many of our Baltimore patients seek eyelid surgery for cosmetic reasons, others request this procedure in order to correct drooping upper eyelids that sag enough to obstruct their vision. Surgery to treat this condition is known as “functional” blepharoplasty.
Additionally, eyelid surgery can:
- Soften puffiness or bags under the eyes.
- Improve the appearance of puffy upper eyelids.
- Reduce excess skin and fat of the upper eyelids that causes you to look tired or sad.

Who’s a Good Candidate for Eyelid Surgery?
As noted above, eyelid surgery can be a suitable solution for both cosmetic and functional purposes. The best candidates for eyelid surgery have moderate to severe eyelid drooping that cannot be addressed without surgery.
Candidates should:
- Be in good physical and emotional health without medical conditions that can impair healing.

Advanced Eyelid Surgery Techniques

Upper Eyelid Surgery
Sagging upper eyelids can make you appear tired or older than you feel and, in some cases, may even obstruct your vision. Upper eyelid surgery involves creating a small incision within the eyelid’s natural crease, allowing your surgeon to remove drooping tissue so you look rested and brighter. The scar typically heals very well and can become virtually unnoticeable over time.
Lower Eyelid Surgery
Lower eyelid surgery addresses puffiness, bags or hollowness beneath the eyes that can contribute to a tired or aged appearance. Through an incision placed either just beneath the lower lash line or inside the eyelid (transconjunctival approach), your surgeon removes or repositions fat deposits and skin below the eyes that cause bags or dark circles. The result is a more youthful and rested appearance, often with no visible scarring.
Brow Lift and Related Procedures
In some cases, a brow lift may be a more beneficial approach to your cosmetic concerns than eyelid surgery. Our surgeons perform brow lifts using a minimally invasive technique often called an endoscopic forehead lift. The procedure involves making small incisions within your hairline and then inserting a thin tube that has a tiny camera attached. Specialized surgical instruments allow your surgeon to reposition the muscle and skin, resulting in an elevated brow and eyelids and a smoother forehead.
Additionally, combining treatments may be a good choice for you. A brow lift may be combined with eyelid surgery or a lower facelift to complete a comprehensive facial rejuvenation in a single operation. Combining eyelid surgery with BOTOX® Cosmetic injections to minimize wrinkling around the eyes is also an excellent way to refresh your appearance without adding additional surgery. You and your surgeon can discuss during your consultation the approach best suited for your specific needs and cosmetic goals.
How to Prepare for Eyelid Surgery
Proper preparation can help to ensure a smooth eyelid surgery experience and optimize your results. Before your procedure, Dr. Chang or Dr. Wang will provide you with detailed pre-op instructions tailored to your needs.
Common eyelid surgery preparation steps include:
- Stop taking blood-thinning medications or supplements (ibuprofen, naproxen, vitamin E, fish oil and others) 1 to 2 weeks before your procedure.
- Refrain from smoking or vaping for at least 2 weeks before and after eyelid surgery.
- Avoid alcohol for at least 48 hours prior to your procedure.
- Arrange for someone to drive you to and from your appointment.
- Do not wear any makeup, false eyelashes or contact lenses on the day of your procedure.
Your Blepharoplasty Procedure
Dr. Chang and Dr. Wang often perform upper and lower eyelid surgeries right in the office under a local anesthetic, which numbs the treatment area while keeping you awake and comfortable. For patients who feel more anxious or are having multiple procedures performed at once, your surgeon may recommend sedation combined with local anesthesia. In these cases, the surgery will be performed at an accredited outpatient surgical center. You and your surgeon will make this decision together prior to the procedure, ensuring safety and comfort based on your unique needs.
The procedure itself typically takes 45 minutes to an hour to complete for either the upper or lower eyelids alone, and a bit longer if both are addressed together. Dr. Chang or Dr. Wang will make precise incisions along the natural creases of your upper eyelids or just beneath the lash line or inside the lower eyelids. Excess skin and fat are carefully removed or repositioned to create a refreshed and more youthful look, without changing the natural shape of your eyes. Once complete, the incisions are closed and you can expect to return home that same day.
Eyelid Surgery Recovery
It is important to arrange for someone to drive you home following your procedure, whether it is performed at our office or a surgical center. While patient experiences vary after eyelid surgery, swelling, bruising and mild discomfort are all normal in the first few days. You may also experience some temporary dryness, tearing or light sensitivity. These symptoms typically peak around day 2 or 3 and should gradually subside.
Most people feel well enough to return to light activities after 3 to 5 days and can typically go back to work in about a week. Makeup can usually be worn again after 10 to 14 days, once healing has progressed. Many patients are surprised at how comfortable and manageable the recovery process is overall.
Eyelid Surgery Results
Eyelid surgery is highly effective at helping patients look more refreshed, youthful and alert without changing their natural appearance. Initial results typically begin to appear within 1 to 2 weeks, once the majority of swelling and bruising has resolved. Final results are typically visible within 3 to 6 after surgery. Because incisions are carefully hidden in the eyelid creases or lash lines, most scars fade to be virtually invisible with time. While eyelid surgery doesn’t stop the aging process, the improvements can last for many years, often a decade or longer.

How much does eyelid surgery cost in Maryland?
The cost of eyelid surgery in Maryland will depend on whether you are treating the lower or upper eyelid or both, as well as other details of your specific procedure that are unique to you. Because of this, our team will be able to provide you with a more accurate eyelid surgery cost estimate at the time of your consultation with Dr. Chang or Dr. Wang. Will my health or vision insurance cover eyelid surgery?
Typically, health and vision insurance will not cover eyelid surgery. There are exceptions to this, however, such as when eyelid surgery will improve the vision of the patient. If your insurance does not cover the procedure, our office can connect you with financing options and offer payment plans that make it more affordable.
How do I find the best plastic surgeon for eyelid surgery near me?
As with any plastic surgery, you want the doctor performing your eyelid surgery to be able to deliver excellent results. While this procedure isn’t rare, it also isn’t amongst the most common surgeries performed every year. This means the key factor in selecting a surgeon is experience.
Look for those who list eyelid surgery on their websites, then schedule consultations with them. Ask about how many eyelid surgeries they have performed and request to see before and after photos. Select the doctor whose results best align with your goals.
Is eyelid surgery painful?
As with any surgery, you can anticipate some discomfort after your eyelid procedure. This is at its strongest directly after surgery. Luckily, due to the significant blood flow to the head and face, healing is fairly quick. Dr. Chang prescribes appropriate pain medications to ensure your comfort after surgery.
Will eyelid surgery correct problems with peripheral vision?
This depends on what is limiting your peripheral vision. If the cause is droopy eyelids, getting surgery will pull them out of your line of vision. However, it is possible that droopy eyelids are not what is limiting your field of vision, or if they are, that they are not the only cause.
Other conditions that could contribute to narrowing your field of vision include glaucoma, occlusions, detached retinas and compression of the optic nerve. If your vision is what has you considering eyelid surgery, see an eye doctor first to ensure none of these are causing you problems.
How long does eyelid surgery take?
The time eyelid surgery requires varies between patients. However, for the majority of individuals, it will take two hours or less. In nearly all cases, this is an outpatient surgery. This means that you will be monitored by the staff for a few hours, but you will go home the same day.
What side effects should I expect after eyelid surgery?
In most cases, people experience swelling, bruising and redness around the incision sites. Also common are light sensitivity, double vision and watery eyes. However, these side effects tend to resolve quickly and are not considered problematic.
As with all surgeries, there is the potential for complications, such as infections, but these are rare. If you follow the directions we provide, you significantly lessen the chances of complications occurring.
Can I lift sagging eyelids without surgery?
Nonsurgical treatments like BOTOX or fillers may offer subtle improvement for mildly sagging eyelids, but they cannot match the results of eyelid surgery. For moderate to severe sagging, blepharoplasty is typically the most effective and lasting solution.
